Home Tele-Monitoring


A Honeywell HomMed® TeleHealth system is a combination of advanced communication technology and health monitoring equipment that is kept in the patient’s home to collect vital information and clinical data regarding his/her condition on a daily basis. This data is transmitted via a standard telephone connection to a central office, where a specially trained clinician monitors and reviews the data being updated.

Home telemonitorIf the data shows unexpected readings, the clinician will contact the patient with instructions to re-monitor, contact the patient’s physician, send a nurse to the patient’s home or contact emergency personnel directly, depending on the severity of the issue.

As hospitals discharge their patients earlier, the need for recovery care in the home becomes more evident. Utilizing this monitoring system brings hospital-grade equipment to the homes of our patients. This gives our patients the peace of mind that irregularities or warning signs of health complications can be detected early to prevent condition deterioration and avoid potentially serious emergencies.

Daily monitoring of a patient at home also helps his/her physician be more aware and pro-active in his/her care, allowing for day-to-day adjustments to medications or the treatment plan.

Prime candidates for HomMed® TeleHealth are those patients who have been discharged from a care facility (e.g. hospital) or diagnosed with chronic or comorbid conditions, such as congestive heart failure, diabetes or high-blood pressure.
